For credit checks, please complete and sign a credit authorization form (PDF provided below) for each applicant requesting a credit check. Once complete fax back to our office fax number indicated at the bottom of the form and add the desired amount of credit checks to the cart below before checkout. Credit Checks are $50 per applicant. The checks will be performed after we receive confirmation of your payment.
